Paperwork

If you’ve already completed your admitting paperwork, good! If not, we’ll give you a few minutes to do so before meeting Dr. Perish. You can download the paperwork here on the web site which will save you time on your first visit but more importantly, allowing you to fill in the form without feeling hurried.

Consultation

The purpose of your consultation with the doctor is to discuss what’s troubling you, your overall health goals and see if chiropractic is a good fit for you. If it looks promising, a thorough examination will help identify the cause of your problem.

History and Examination

In order to determine what your actual problem is, the doctor will go through some questions related to your condition. Then, several specialized tests will be performed to determine exactly what’s going on.

X-ray Studies

Your condition may require us to take x-rays. We never take x-rays which are not needed however they may be necessary to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Same Day Treatment

Unless there is reason otherwise, you will receive treatment on your first visit. Our initial goal is to get you feeling better as quickly as possible. If for any reason Dr. Perish does not feel you’re in the best place for you, he will make appropriate referral suggestions.

Home Instructions

Before leaving, we will give you home instructions which can help speed your recovery.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Most patients are seen within 1-2 days. During your second visit Dr. Perish will go over case and treatment options.
